Heather Powers, RSW, PMP

Hi! My name is Heather and I am a registered social worker, and Project Management Professional (PMP) in Ontario and British Columbia. I have spent the last 16 + years working in a variety of non-profit organizations in Canada and around the world. I have spent time in schools, community centres, churches, boardrooms, program spaces, clinical spaces, and homes. This breadth of experience allows me to bring a holistic perspective to the organizations I work with. I apply my experience and knowledge of trauma, human behaviour, community development, strategic planning and implementation, and organizational leadership to support organizations to serve their communities to the best of their ability. Let’s work together.

With the unique experience of working in both community and clinical spaces, I have learned that one cannot exist without the other. As humans, we need each other. Relationships and connections are what make life meaningful; but, we also need space as individuals to reflect, process, and learn. In Yellow Pine C&C, I bring my two worlds together. I am here to hold space for individuals as we journey together through whatever it is that life has thrown at you.

Life is messy and can be unpredictable. But, as we journey together, we can find meaning, vitality, and wholeness.

Jeremy Horne, MSW

Jeremy has spent his career dedicated to community development, leadership, and mentorship. Jeremy brings deep expertise in organizational development through his experiece as the founder of “Adventure4change”, a non-profit organization that serves marginalized communities in the Waterloo area, including refugees and low-income families. Through this work, he helps people gain access to opportunities that empower them to flourish in all aspects of life.

His career is a testament to his passion for helping others. From his early work in mental health to his varied leadership roles within the not-for-profit sector, he has consistently guided people toward better lives. Jeremy believes in supporting the next generation by equipping them with tools and resources for long-term success, especially in marginalized communities where opportunities are limited.

Jeremy joined YPCC in 2023 as an expert leadership and organizational development coach. His commitment to supporting the growth and development of leaders ensures organizations thrive.
